Initial patients seeking to obtain a Utah Medical Cannabis recommendation must be seen by our QMP in person.
Note: We are accepting patients 21 years of age and older at this time.

RENEW AN ACTIVE CARD
This service is for patients seeking to renew an active Utah Medical Cannabis Card.
We renew active cards from ANY provider.
$100
Note - One time $50 transfer fee for New Patients
RENEW AN EXIRED CARD
This service is for patients seeking to renew an expired Utah Medical Cannabis Card that is more than 2 weeks over the expiration date.
We renew expired Utah Medical Cannabis Cards from ANY Provider.

HOSPICE OR TERMINALLY ILL PATIENT
This service is for patients with a terminal illness and/or on hospice.
A Qualified Medical Professional (QMP) that is registered with the Department of Health will do a home visit at no additional charge.
